If a medical report had been submitted to either the ICA or the Ministry of Manpower not more than 2 years ago, the applicant will need only to complete the declaration portion of the attached medical form.

根据你的上文，不需要，因为家属2年内已提交给ICA或MOM体检报告。不过需在所附的体检Form进行有关披露。严谨一点的话，递交时跟officer再confirm一次。
